PRESCRIPTION FOR MONOFOCAL LENSES:

You will see that in your inform of graduation appears three numerical data, corresponding to three concepts: SPHERE, CYLINDER AND AXIS.

If you have clearly identified (upload a picture of graduation well done), incorporate each data in the appropriate box on our form . In the case they are not well identified (for example, if you encounter a succession of data without being told which concept belongs to each) then you know that the data is always written in the order in which we did. In other words, the sphere first, then the cylinder and finally the axis. So you have identified them and can add them into their appropriate boxes. Never forget to incorporate both number and the plus (+) or minus (-) you get ahead, because without it we could not do your lenses.

In your graduation data should appear in addition identified with the initials D.P., corresponding to the Distance Guardianship. If so, add it in the box.
